What it does

Written by AI from the README

Controls what happens when you double-click items and attachments. It opens the main article PDF ahead of supplementary files, previews Markdown, CSV, JSON, text and zip files in Zotero tabs, and sends files to apps you choose with Option + double-click.

Double-clicking a parent item can open a supplementary PDF, and other file types fall through to whatever app your system picks. This gives you predictable routing without changing Zotero's global reader settings.
